At Nanar Hovasapian OD, we value your time. In an effort to save you time in our office and make your vision exam one of the easier obligations in your life, please fill out the form before coming into the office for your appointment.

All patients can expect to be treated respectfully throughout the course of their visit. Each patient will receive a thorough exam from our highly experienced doctor and can also expect to have their needs met by our optical team.

It would be helpful to bring any current eyeglasses (even if rarely used), and any recent contact lens boxes or prescriptions. If referred from another doctor, please bring your referral form. Provide a current, non-expired form of identification (driver's license, passport, etc.) and any insurance cards. You may also fill out a Release of Records form in our office if you'd like previous records transferred.

We are in-network providers for VSP and Medicare. Our team will check your benefits for you and explain them in detail upon request. You will also be expected to pay what is determined as not payable by your insurance for services and materials at the time of the services.

We accept Visa, Mastercard, Discover, American Express, Apple Pay, Samsung Pay, and Google Pay. Payment for all services is due at the time services are rendered, including co-pays. Payment for contact lenses and/or glasses must be paid in full at the time of the order.
